About Us
St. Andrew’s Community Hospital (SACH) is a service of St. Andrew’s Mission Hospital.
Established in 1992, SACH was the first community hospital set up to provide affordable intermediate medical care. We are focused on providing rehabilitation and sub-acute care to adults and the elderly; including those with dementia or needing palliative care. Our outpatient services include day rehabilitation, community therapy, primary healthcare, home healthcare and other senior care services.
Community hospitals play an important role in providing continuing care for patients upon their discharge from acute care hospitals. SACH aims to promote recovery and help patients integrate back into the community after their discharge.

St. Andrew’s Mission Hospital (SAMH), whose services include that of St. Andrew’s Community Hospital (SACH), is governed by an Act of Parliament – St. Andrew’s Mission Hospital (Chapter 376) (Original Enactment: Ordinance 42 of 1934). A Management Committee provides SACH strategic guidance and direction to steer the hospital towards realising its goals and objectives.

- St. Andrew’s Community Hospital (SACH) aims to comply with the requirements of the Personal Data Protection Act 2012 and apply its obligations to our processes to safeguard your personal data. This notice (“Data Protection Notice”) aims to inform you how SACH manages the personal data under its possession or control.
- Personal Data refers to data, whether true or not, about an individual who can be identified from that data; or from that data and other information to which the organisation has or is likely to have access.
- The personal data SACH collects, uses and discloses include but are not limited to:
- name, NRIC, passport or other identification number, photos, biodata;
- telephone number(s), mailing address, email address;
- family members’ information, employment history, education background, bank account information, credit card information, medical records.
- SACH only collects, uses or discloses personal data appropriate for the intended matters you engage SACH for. Generally, SACH collects, uses or discloses personal data:
- when you are referred to or admitted to SACH services or our authorised service providers;
- when you apply to and/or volunteer, intern or work for us;
- when you apply to and/or donate to us;
- when you register for and/or attend any of our events;
- when you perform a financial transaction with us;
- when you submit a feedback and/or a request for contact;
- when you access our facilities and/or website;
- when you apply to be our society member
- other purposes which are reasonably related to the aforesaid.
- SACH will notify and obtain consent for collection, use or disclosure of personal data. If you provide the personal data belonging to others, you warrant that you have informed the individuals of the purposes for which we are collecting their personal data and that they have consented to your disclosure of their personal data to us for those purposes.
- For the purpose of disclosure, SACH will seek relevant consent for the purpose in delivering relevant service(s). SACH being part of the public healthcare and social services system, will operate under a “deemed consent” framework for collection, use and disclosure of data for the delivery of our services. This includes accessing the use of relevant patient data in the National Electronic Health Records (NEHR). Parties to which the personal data may be disclosed to include relevant public agencies, agencies acting on behalf of public agencies, relevant healthcare and social care service providers, welfare funds, approved private medical insurers, etc.
- If there is a purpose for use and disclosure beyond what has been consented for or the purpose you engage SACH for, we will notify and obtain new consent from you.
- You may (i) withdraw consent, (ii) request for access, or (iii) request for correction of your personal data given or deemed to have been given. SACH will facilitate such request and advise you accordingly how the withdrawal may affect our provision of service to you. We will also advise you if any charges apply for your request of access.
- SACH will cease to retain personal data if the legal or business purpose(s) for which data was collected is no longer valid.
- SACH maintains appropriate security measures to protect your personal data from unauthorised access, collection, use, disclosure, deletion, disposable, loss and other risks. SACH website may contain links to other websites operated by third parties independent of SACH. SACH is not responsible for the data protection practices of such websites. We encourage you to learn about their data protection practices.

St. Andrew’s Community Hospital (SACH) is committed to providing our patients and visitors the best possible care. Tell us if our service falls below expectations.

- Under Section 6 of the Protection from Harassment Act (POHA) 2014, a person can be convicted of a fine not exceeding $5,000 or imprisonment for a term not exceeding 12 months or both, for using abusive or insulting language on *public service workers when carrying out their duty.
- Give us a safe environment to deliver the best possible care. Under the POHA 2014, it is an offence to abuse healthcare workers. Examples of abuse are:
- Any threatening, abusive or insulting acts that are likely to cause our staff to feel harassed, alarmed or distressed, or any acts that are likely to cause our staff to believe that violence will be used against them
- Stalking our staff
- Publishing any information that may be used to identify our staff with the intention to cause harassment
- Our patients and staff have a right to privacy. No photography and video recording are allowed.
- SACH will respond firmly to protect our staff, including – where necessary – the reporting of the abuse incident to the Police for investigation under the Act.
*A healthcare worker is a public service worker.

Background Information
St. Andrew’s Community Hospital (SACH) is inviting vendors to submit proposals for the Provision, Installation, Configuration, Testing, Implementation and Maintenance of Data Driven Decision Making System for the community services under SAMH group.
The service provider is to provide an End-to-End (E2E) solution that can extract, transform and load (ETL) data from various application systems into a central repository with normalized schema and the capability to interface with data analytics tools to generate dashboards and reports for analytics purposes. The system shall utilize best practice processes and workflows to meet the needs of SACH.
SACH is sourcing for a solution to support the project implementation and delivery. The solution could be made up of a suite of well-integrated systems.

- The expected project duration is 6 months from the date of project award.
